Student Health Form
Print and fill out a Health Form for your chorus student. This is MANDATORY before your student can stay for any after-school activities or go on any trips. If your insurance information or personal information changes, please print this again and fill out the entire form, updating the changed information. This form should be completed and turned in at either the first Booster Meeting or on Uniform Day.
Volunteer and Student Information Form
This is a two-sided form. Please print and fill out both sides at the beginning of each year to let us know what you are interested in helping with, and to give us all the contact information we will need. This form should be completed and turned in at the first Booster Meeting or on Uniform Day. If your schedule or your interests change, update your form and give it to your chorus student to turn in to Mr. Ehrlich.
Uniform Rental Agreement 2010-2011
This form will be used when assigning uniforms on Uniform Day and lists the fees for each item. Your student should arrive at Uniform Day with a check payable to "W.T. Woodson Choral Boosters". Be sure to write your student's name on the check. If financial arrangements are required, please speak with the choral director (703-503-4667) prior to Uniform Day. Unless previous arrangements have been made with the choral director, students can not be permitted to take home a uniform without payment.
